Patient Portal

Patients can now access our Patient Portal through Kareo. This portal will allow patients, both current and new, to send us messages, request appointments, view recent test results (from Quest & Labcorp only) and update their information. Contact someone at the office to invite you & then follow the instructions in the email sent to access the portal!

Before your appointment

Returning patients: If you have been a patient of IHC  before but not been in within the last 2 years, please bring in relevant records, update your medical information, and all your medications and supplements. If you have moved or changed phone numbers, pharmacies or anything else please let us know so we can better serve you!

If you have chronic health problems, it will help us serve you and save time if you also complete the Medical Symptom Questionnaire and the Nutrition and Lifestyle Journal along with any others that seem appropriate for you and your symptoms. Additional forms include:
